When completing this form, please be as thorough as possible. The information you provide will be used in the evaluation process.
In order to be eligible for consideration, companies must meet the following criteria:
• Company must have been established prior to July 1, 2018.
• Company must have a headquarters office based in the United States.
• Must have at least 10 full-time employees.
• Must complete submission form before September 3, 2019 11:59PM (PST).
• Incomplete forms may be subject to disqualification.
Upon confirmation of eligibility, companies may be asked to submit a selection of high-resolution company photographs that best reflect the brand and working environment. Photography guidelines will be provided.
